Physicians Pivotal in Administrative Leadership at Health Systems and Hospitals
The Story
In a recent roundup, HealthLeaders analyzed how physicians can play active roles in the administrative leadership of their organizations. For example, while Allegheny General Hospital is clinician-led with a practicing hospitalist serving as president, RWJBarnabas Health has nurse and physician leaders spearheading quality, safety, and patient satisfaction projects. Providence Newberg Medical Center invites physicians to serve as chairs for clinical departments, while Yuma Regional Medical Center appoints physicians as executive medical directors to oversee departments such as primary and surgical care.

Direct Axillary Artery Cannulation: Safe, Expeditious, and Hemostatic
The Story
CTSNet has recently shared an innovative procedure for aortic arch repair, acute aortic dissection repair, or complex redo aortic or cardiac cases. The direct axillary artery cannulation involves retraction of the distal AX via vessel loops to create a slope, which allows for the direct cannulation. While the potential for vascular injury and iatrogenic dissection have been concerns for this approach, the authors share that a previous study shows the risks are low. Further, no arm ischemia has been experienced by the study’s authors in more than 500 direct axillary artery procedures.
     
What You Should Know
Based on the authors’ findings, the benefits of the direct artery cannulation technique appear to be quite compelling. Compared to the side-graft technique, this approach may lead to reduced bleeding during cardiopulmonary bypass and boasts greater expediency. Additionally, this procedure requires no need to control distal flow in the ipsilateral arm, and no foreign body left upon the procedure’s completion.

Mitigating risk: How the depth of device implantation impacts LAAO outcomes
The Story
In the first study to look at left atrial appendage occlusion (LAAO) device implantation depth and its relation to device-related thrombosis (DRT), researchers analyzed data from more than 1,300 patients who underwent the procedure across Europe or Canada from 2012 to 2020. The majority of patients (64%) were men, and the median age was 74.9 years old. They separated patients into groups based on the depth of the implantation related to the pulmonary ridge (PR), and identified a stepwise increase in DRT incidence at further depths.
     
What You Should Know
The study authors’ findings suggest that LAAO implantation depth is an important risk factor for DRT. The risk was primarily observed beyond 5 mm, with the highest risk occurring at a depth of 15 mm or more from the PR. With these results in mind, the authors encourage surgeons to aim for proximal device implantation as the optimal target for the procedure.

Sharing Time and Expertise
While financial donations are important to non-profit organizations, the time and expertise of organizational leaders can be equally, if not more valuable. When actively engaging with non-profits, leaders share their knowledge, skills, and networks to drive meaningful change. Their experience in strategic planning, management, and problem-solving can promote competent governance, clarify expectations for the CEO or Executive Director, and ultimately, help to lay the foundation for organizational mission, vision, and values.

Networking and Relationship Development
Board service has not only allowed me to give back but has also provided a place for me to connect with a broader community of change-makers, creating a powerful network of social and professional support. Non-profit boards often comprise a diverse group of individuals from various industries, backgrounds, and professions. Collaborating with fellow board members has allowed me to establish meaningful connections with like-minded people dedicated to making a positive impact.

Personal Fulfillment and Growth
Beyond the external benefits, I have found personal fulfillment and growth through my involvement in non-profit organizations. Many leaders discover a sense of purpose by dedicating their time and resources to causes they are passionate about. This can lead to a greater sense of satisfaction and meaning in their lives, beyond the pursuit of financial success.
